Geriatric Assessment and Intervention Network (GAIN) clinic

Carolyn graduated from the University of Toronto, Faculty of Pharmacy and completed a hospital pharmacy residency at Mount Sinai Hospital. For the past 30 years she has worked at Southlake Regional Health Centre in Newmarket Ontario, the last 15 years providing patient care and education in the hospital’s Arthritis Program, an ambulatory outpatient clinic. In 2017 Carolyn was the first pharmacist to complete The Arthritis Society’s course, Clinical Practice Skills in Inflammatory Arthritis. She is also a board certified geriatric pharmacist by the Board of Pharmacy Specialties and works part time in the Geriatric Assessment and Intervention Network (GAIN) clinic in Lindsay, Ontario, providing medication assessments including in-home visits. Carolyn is a recipient of the CSHP (Canadian Society of Hospital Pharmacists) Distinguished Service Award, is a CSHP Fellow, and a CSHP Past President.
